In MOFI, you can fail by entering the volcano without enough inventory space for a lava key.
You can also fail by buying too many lamp refills.

In the bridge area when you talk to the purple hat guy, if you stand in just the right place, a section of the void will appear under you while you're talking, therefore killing you.

The list allows the cheat code (ctrl + f12) to be used (unlike wall-blinking and the "other code"). One can cheat on that adventure.

Here's another one:
When you find the stinker's lucky gem (on the way to the Foggy Mountains), you can get back to her house before she does. When she arrives back home, she will trap you.
