First I made some textures,because I used 3D models for that image.
Next I made the scene with a normal 3d program.
Export an image and make the background color transparent
I added The dialog box .
Added the background and saved as png,because jpg quality isn't very good and bmp is too big.

One unlucky Chomper's teeth were found to be slightly misshaped. Thankfully, Stinkers have already figured out ways to fix crooked teeth, and he's now nearing the end of his orthodontic treatment.
Scene created fully in WA Editor, except for the background and blur (photoshop). Yes, the editor lets me look from such angles.
