Like I said earlier, at least Pat's updating us on a somewhat frequent basis about how things are going.
Some companies don't at all, though they usually have their reasons - as I alluded to earlier, VALVe's a good example. Everyone's still waiting for news on Half-Life 2: Episode 3, or Half-Life 3, or whatever it's called these days.
Episode 2 came out in 2007, and they haven't said anything since. According to their CEO, the reason is because they don't want to disappoint people by changing the game's direction between now and when it comes out, which they've apparently done several times.
A lot worse than what we've got with PoTZ/WA3, because we're getting updates on it.

Those are usually either officially released gameplay clips to drum up hype, or people recording themselves playing the demos at E3.
In some rare cases the game gets leaked early, that's another possibility (most recent example I can think of is another Valve one, Portal 2 was leaked a few days early for the Xbox 360).

Not if people have patience. WA was released sometime in February 2007. MOFI was released a year later in December 2008. That means that in the 2007-2008 time period, 2 humongous games were released, and not a single drop in quality in them was found.
